It depends on what the cardiologist needs to do, but at a minimum, they would need to listen to your heart with a stethoscope, which they would do under your top. If you need an electrocardiogram or echocardiogram, which is kind of like an ultrasound of your heart, you would need to remove your top and bra and probably put on some sort of hospital gown but your underpants can stay on.
